Job Description
The Physical Therapist will effectively evaluate, diagnose, and identify movement impairments and functional limitations in patients. This role involves developing personalized treatment plans aimed at alleviating pain, restoring movement, and preventing disability, while also focusing on maintaining mobility.
Essential Functions and Responsibilities
- Conduct comprehensive patient evaluations by obtaining medical history, performing relevant system reviews, and administering specific tests to gather necessary data.
- Create individualized care plans and select treatment techniques based on evaluation outcomes and physician orders.
- Perform ongoing assessments of patients to monitor their response to treatment and adjust plans as needed.
- Initiate and manage discharge planning from the first visit through the completion of treatment.
- Supervise and delegate tasks to ancillary staff and students in accordance with state regulations and department policies.
- Ensure clear, accurate, and legible documentation is completed within the required timeframes, adhering to department policies, accreditation standards, and insurance regulations.
- Communicate effectively with patients, families, caregivers, and healthcare team members to maximize the benefits of care through the AIDET framework.
- Assess and address the educational needs of patients and their support systems, providing appropriate education as required.
- Engage in ongoing professional development through participation in educational programs, workshops, and by reviewing current literature relevant to the practice.
- Contribute to quality improvement initiatives within the department.
- Maintain a safe environment for patients, visitors, and staff, complying with approved safety standards.
- Operate within the boundaries of practice defined by state regulations.
- Perform additional related duties as assigned.
