JOB DUTIES/RESPONSIBILITES: Conducts psychosocial assessments to document history and identify preliminary issues for treatment focus by interviewing the patient and their family members. Coordinates care of assigned patients throughout their stay at SM and participates in patient rounds to plan and evaluate individualized safe and timely patient discharge needs. Coordinates referrals for these services and utilizes interpersonal skills to maximize excellence in customer service, safe patient care, and professionalism. Develops and coordinates an individualized discharge plan for the patient by utilizing team and chart information to determine the patient s aftercare needs. Works with Discharge Planner to develop, implement and document a discharge plan established with the patient per JCAHO standards and with attention being focused on progressing toward discharge. Attend Treatment Team Meetings as determined by the Director of Clinical Services Demonstrates current knowledge about full range of discharge options/community resources and utilizes them in the patient’s discharge plan. Identifies and assesses family or community resources such as, group homes, or mental health practitioners to meet patient s aftercare needs. Reports to community/state agencies any patient issues required and represents facility in any legal proceedings. Provides group and family process-oriented therapies as well as conducts educational and other didactic groups for patients and family members using various professional treatment modalities. Schedule Notes:Day – Mon – Frit- 13 weeksSkills: REQUIRED KNOWLEDGE, SKILLS AND ABILITIES: Experience with age-specific inpatient setting preferred Experience in taking psychosocial history and case management Knowledge of developmental psychology of adolescent and adults and have the capacity to identify pertinent clinical information Possess knowledge of clinical issues related to mental health patients and their discharge planning New hires are required to attend 1 full week of New Employee Orientation on day shift, normally 8:00 am 4:30 pm Monday through Friday Must successfully pass background check (includes criminal history, Child Abuse and Neglect Registry, and ABI/FBI fingerprint check) and drug screen Required PSYCHIATRIC/BEHAVIORAL CASE MANAGEMENT AdditionalEducation: QUALITFICATIONS: Master s degree in social work Current licensure in the State of Nevada; also acceptable licensure: CSW-I, LCSW;Languages: English Read Write Speak English(Speak , Read , Write)Job Category: Clinical Allied Travel Guaranteed Hours: 40.00 Contract Weeks:89
